As you may be aware, Target has confirmed that there was unauthorized access to payment card data that may have impacted credit and debit cardholders who made purchases between November 27, 2013 and December 15, 2013 in its U.S. stores.
At this time, Hoya FCU has not received information that any of our cardholders were among those that were compromised. However, the investigation is ongoing and as of December 20, VISA has not yet released its notification to its card issuers relating to this breach. Should we receive notification from VISA that any of our cards have been compromised, we notify you and issue a new card to you automatically.
Hoya FCU takes protecting your security seriously and recommends, if you made purchases at Target during this time period, you request a new card. We will order you a new card and you will continue to have use of your existing card while you await your replacement.

Upon completing the form you can fax it back to the Credit Union at (202)687-6920 or submit it in person. A new card and PIN will be mailed to you. Generally, cards are received between 7 to 10 business days, but due to the overwhelming demand on card replacement, receipt of your new card may take longer. In any event, a Hoya FCU staff member will contact you upon receiving your card request.
